Failing CMMC Should Be Rare—and Preventable

CMMC is often discussed as if it’s a surprise inspection that organizations either “pass” or “fail” depending on how an assessment day goes. That framing misses what the program actually is.

CMMC is a known standard, with published controls, documented assessment objectives, and an explicit expectation that an organization validates readiness before scheduling an assessment. In that sense, it’s closer to an open-book test than a pop quiz. The questions are known. The grading criteria are known. And preparation isn’t just allowed, it’s required.

That doesn’t make CMMC easy. It does mean that outright failure should be rare.

Where CMMC Assessments Go Off the Rails

When organizations fail a CMMC assessment, it’s rarely because a control was misunderstood or because an assessor applied unexpected criteria. More often, the failure happens upstream, before the assessment ever begins. Recent surveys of defense contractors reveal that a significant portion still lack robust governance and tracking for controls — underscoring how often failures stem from process gaps rather than the standard itself.

Common patterns include:

  • Readiness treated as a checkbox.
    Completing a gap assessment or checklist is not the same as validating readiness. Evidence, ownership, and decision paths matter more than completion status. A gap assessment or checklist alone isn't enough; assessors look for evidence that's current, mapped to actual operations, and shows ongoing ownership, not just a one-time exercise

  • Scheduling pressure overriding reality.
    Contract timelines and customer demands can push organizations to declare themselves ready before they actually are.

  • Known gaps deferred without formal decisions.
    “We’ll fix it later” isn’t remediation unless it’s documented, owned, and tracked.

  • Risk ownership stopping at “the security team.”
    If no single person owns a control or a POA&M, the organization doesn’t really own the risk.

None of these are technical failures. They’re process failures.

What “Failing” Really Means

To be clear, discovering new gaps during an assessment is normal. No readiness process is perfect, and assessors aren’t looking for flawlessness.

What sinks organizations is something different: walking into an assessment with unresolved, known issues that were never formally addressed, accepted, or planned. That’s not bad luck. That’s a breakdown in readiness discipline.

A common contributor to this breakdown is the misuse of POA&Ms. At Level 2, POA&Ms are not a general-purpose buffer for unresolved controls; they are limited in scope and explicitly time-bound. Yet organizations still rely on them to paper over gaps in well-documented requirements, such as cryptographic implementations that depend on validated solutions rather than configuration intent alone. These aren’t edge cases. They’re widely discussed, readily testable, and discoverable well before an assessment is scheduled.

What Real Readiness Looks Like

CMMC readiness doesn’t require eliminating all risk. It requires proving that risk is understood and managed.

In practice, that means:

  • Every control is mapped to evidence that actually exists

  • Every gap has a documented remediation plan or risk acceptance

  • Every POA&M has a named owner and a realistic timeline

  • Leadership has visibility into known gaps and has formally approved how each one will be addressed, remediated, or resolved where allowed

When those conditions are met, assessments tend to be uneventful, not because the organization is perfect, but because it’s honest and prepared.

A Preventable Outcome

CMMC isn’t designed to surprise organizations or trick them into failure. It’s designed to verify that a known set of practices are in place and functioning. Organizations that treat readiness as an ongoing discipline — rather than a last-minute scramble — consistently see smoother assessments.

When an organization fails, it’s often because the readiness process was rushed, softened, or skipped—not because the standard was unreasonable.

In that sense, failing CMMC really is like failing an open-book test. The outcome usually isn’t about the material. It’s about whether the organization chose to prepare honestly before it sat down.
